Make a complaint


By clicking submit you agree that: 

  • an Aviva representative may contact you via telephone, email, SMS or letter, in order to review and discuss your complaint;
  • if you are submitting a complaint on behalf of someone else; you confirm that you have full legal authority to act on behalf of that other person; and 
  • Aviva may:
    • obtain personal information from you and other sources;
    • disclose personal information to such other sources; as necessary to investigate and resolve your complaint. Other sources are third parties that may provide information that may be relevant to the investigation and resolution of your complaint. Other sources may include, without limitation, brokers, clinics, other insurance companies, WSIB, experts and engineers etc. The personal information to be collected and the third parties that may be contacted will vary in accordance with the nature of the complaint. Aviva will collect, use and disclose personal information in accordance with Aviva’s privacy policy.
